Otter 257 was delivered to the United States Army on 24th April 1958 with serial 57-6122 (tail number 76122). It was one of sixteen Army Otters delivered from Downsview to Addison, Texas for work to be done on them by Collins Radio Corporation. Most of these Otters were then delivered to Europe, including 76122 which by January 1962 was serving with the 110th Aviation Company, Boscomantico AAF, Verona, Italy, part of SETAF. In January 1963 it was transported back to the United States and was assigned to the Aviation Section at Fort Ord, California in the operational support airlift role. In February 1966 it returned to Europe and arrived at Coleman Barracks, Mannheim, Germany for depot level maintenance, after which it was issued in May 1966 to the 2nd Military Intelligence Battalion at Sembach Air Base, Germany.

In November 1967, 76122 returned to Coleman Barracks from where it was transported back  to the United States and then onwards to Vietnam, where it joined the 2nd Signal group in May 1968. It continued to serve with this unit until November 1969, when it was assigned to the 18th Aviation Company. It ended its military career with the 18th Aviation Company when that unit disbanded in March 1971, and was flown to Vung Tau and put in charge of the 388th Transportation Company, which was responsible for the disposal of all Army Otters in Vietnam. It remained in store at  Vung Tau until scrapped there in July 1971.
