Otter 352 was delivered to the United States Army on 20th January 1960 with serial 59-2214 (tail number 92214). It was assigned to the 1063rd Aviation Company, an Army National Guard unit based at Waterloo, Iowa and carried 'Iowa National Guard' titles. In October 1961, this unit was upgraded to Active Duty status and sent to Fort Riley, Kansas to operate Otters which the 18th Aviation Company had left behind when it deployed to Vietnam. 92214 accompanied the 1063rd to Fort Riley and was operated alongside the former 18th Aviation Company aircraft.

In July 1962 the 1063rd Aviation Company returned to Waterloo, Iowa, as did 92214. It flew for this unit until January 1964 when it was transferred. Unfortunately the subsequent units which operated this Otter have not been identified. In December 1966 it went to an Army Guard unit. From November 1968 it was flying for an Army aviation school. It met with an accident in March 1969 and was broken up for spare parts the following month.
